* Catch 22 Form*  all applicants must complete the Catch 22 form as part of their application packet from October through December 2024.  ALL APPLICANTS MUST COMPLETE A CATCH 22 FORM.

The KY Board requires that all Foreign Educated Applicants for licensure have their education evaluated using the "Coursework Evaluation Tool" by a credentialing agency.
