The Kentucky Board of Physical Therapy protects the public by ensuring that licensed physical therapists and physical therapist assistants provide safe and effective care to patients.
Renew your license or credential for the 2025-2027 Biennium. This link will be active until March 31, 2025.
The following information on licensees is available free of charge: Licensee Name, License Number, Expiration Date, Effective Date, Status and Sanctions.
File a complaint with the Board. Complaints can be submitted anonymously.
Accepted forms of payment for first time applicants include: cashier’s check, certified check, or money order made payable to Kentucky Board of Physical Therapy.
Credential holders shall notify the board within 30 days of any change in home address, phone number, email address, or physical therapy worksites.

To begin a search, select the State where you believe an individual holds a Compact Privilege and enter the individual’s Last Name.
Successful completion of this exam is required for initial licensure and for renewal of all physical therapists and physical therapist assistants. For renewal, this exam must be completed by March 31, 2025.
IPTPC is a program to promote the early identification, intervention, treatment and rehabilitation of PTs and PTAs who may be impaired by reason of illness, alcohol or drug abuse.
