Simply complete the payment form, selecting “Verification” from the purchase menu. Complete the State or Entity box with the State or Entity where you want the verification sent. We will send out your verification within 24-48 hours of receiving your request and payment.

The official verification will contain the following items:
Name, credential, license number, date of issue, status (active, suspended, lapsed, revoked), expiration date, NPTE date, PT/PTA education institution, graduation year, address of record and whether the license has been disciplined. If the license has been disciplined, a copy of the final order is included with the verification.
