Criminal Background Check

The criminal background check is an essential part of employment for physical therapists in Kentucky.​

State and Federal Background Check Requirement for Licensing with the Kentucky Board of Physical Therapy (“the Board”)

  • Every new applicant will be required to submit fingerprints to the Kentucky State Police (KSP) and pay for a state and federal background check. The cost is $51.25 ($20-state fee, $13.25-FBI fee, $18-IDEMIA fingerprinting fee.) The Board does not have coupon codes/Authorization Codes for this service.
  • Applicants wishing to get fingerprinted in Kentucky will schedule a fingerprinting appointment with one of the IdentoGO agencies authorized by the Kentucky State Police to collect fingerprints for background checks.
  • The applicant will use the Kentucky Board of Physical Therapy Service Code 27GK3Q to schedule an appointment online through this link: IdentoGO scheduling for Kentucky
·         You must finish the IdentoGO registration process online to be fingerprinted. You will receive an email or confirmation number when registration is complete. Please see the Fingerprint Service Code Form which explains what type of identification documents you are required to bring with you to your fingerprint appointment. Your name must match the documents you provide to verify your identity.

  • Remember the phone numbers and/or email address provided to IdentoGO, as they will be used to retrieve your information during your in-person enrollment.

If you reside outside the state of Kentucky, you may choose either digital fingerprinting submission at an IdentoGo location (Livescan) OR you may choose to submit a fingerprint card (Cardscan.) Both require online registration and payment. The IdentoGO customer service call center number is (844) 543-9714.

Please note: Livescan outside KY (with a few limited exceptions in TN) will be an additional $39.95 charge.
            1.      Pre-enroll for Livescan Processing at https://uenroll.identogo.com/ by selecting the option to locate the nearest Out-Of-State location for Livescan Fingerprinting Services.

2.    Identify the closest IdentoGO location you would like to get printed at via the zip-code lookup.  If no location is available within 100 miles or you do not wish to visit the identified location, there is an option to switch to Cardscan Processing. https://www.identogo.com/locations
3.      Select an appointment date and time and complete the pre-enrollment steps.
4.      Complete the enrollment and printing at the selected location.
Cardscan:
1.    Pre-enroll for card scan submission at https:/uenroll.identogo.com/.  The $51.25 processing fee will be collected during the pre-enrollment process (via credit card). There is no additional charge. A pre-enrollment confirmation page will be provided once registration is complete.
2.    Obtain fingerprints on FBI (FD-258) fingerprint card and complete all applicable personal information fields on fingerprint card.
3.    Print and sign the completed pre-enrollment confirmation page, which includes the barcode printed on the top right of the page. Mail the signed pre-enrollment confirmation page and the completed fingerprint card to the mailing address provided or Kentucky State Police Records Branch, 1266 Louisville Road, Frankfort, KY 40601.
4.    IDEMIA requires only the completed fingerprint card (including SSN if required) and the UEP confirmation page to complete the cardscan submission. Applicant packets submitted that do not include a signed UEP confirmation page will be returned TO THE APPLICANT.
Additional Information

  • The Board will not be collecting or submitting fingerprint cards or payment on an applicant’s behalf. The Board has no ability to schedule appointments for applicants.
  • We have been advised that it can take up to 6 weeks for the results to be returned to the agency. Therefore, we are advising exam candidates to submit their fingerprints at least 8-10 weeks prior to their anticipated NPTE exam date.
  • If an applicant has no criminal history returned to the Board, their application proceeds as before.
  • If an applicant has criminal history returned to the Board, she/he may be contacted to provide further information to the Board for consideration.  The Board will then determine how to proceed.
  • An applicant can check the status of their fingerprinting through the IdentoGO website. If you provide an email address during registration, IdentoGO will notify you via email.
  • Applicants may reschedule appointments through the IdentoGO website.
  • The Board is not able to provide the criminal history report to anyone, including the applicant. If the applicant wants a copy of the report, requests may be submitted to KSP and FBI.  Similarly, there is a process with KSP and the FBI for contesting and/or correcting inaccurate information on the report.
  • To obtain your own copy of your FBI criminal history report: https://www.fbi.gov/services/cjis/identity-history-summary-checks.  Several different options are available. You will have to get fingerprints taken again.​
  • To obtain your own copy of your Kentucky criminal history report: Fill out and submit the “KSP Criminal History Review Request” form.  The fee is $20.
  • Criminal background checks are valid for 6 months from the date of their receipt by the Board. Therefore, it is important for an applicant to be aware of timing with regards to submitting their background checks, taking the NPTE, etc.
  • An applicant will not be approved for testing with FSBPT or licensure without a completed state and FBI background check.
FINGERPRINT USE AND CHALLENGE INFORMATION
The Kentucky Board of Physical Therapy has the legal authority, KRS 327.310, to require a criminal background investigation of an applicant for licensure by requiring fingerprinting of the applicant.
 In accordance with FBI policy, please read the following information regarding the use and dissemination/restriction of the fingerprint report obtained from the FBI:
     “Under provisions set forth in Title 28, code of Federal Regulations (CFR), Section 50.12, both governmental and nongovernmental entities authorized to submit fingerprints and receive FBI identification records must notify the individuals fingerprinted that the fingerprints will be used to check the criminal history records of the FBI. Identification records obtained from the FBI may be used solely for the purpose requested and may not be disseminated outside the receiving department, related agency, or other authorized entity. If the information on the record is used to disqualify an applicant, the official making the determination of suitability for licensing or employment shall provide the applicant the opportunity to complete, or challenge the accuracy of, the information contained in the FBI identification record. The deciding official should not deny the license or employment based on the information in the record until the applicant has been afforded a reasonable time to correct or complete the information, or has declined to do so. An individual should be presumed not guilty of any charge/arrest for which there is no final disposition stated on the record or otherwise determined. If the applicant wishes to correct the record as it appears in the FBI’s CJIS division records system, the applicant should be advised that the procedures to change, correct or update the record are set forth in Title 28, CFR, Section 16.34.”
